Ontario Health atHome is a provincially-governed organization dedicated to coordinating high-quality, integrated in-home and community-based care for thousands of patients across Ontario. Their mission centers on supporting patient health and well-being through collaboration with health system partners to ensure residents can recover at home or transition safely to long-term care. With a massive workforce of over 9,200 dedicated professionals, the organization provided essential care to more than 680,000 patients during the 2024-2025 period. The clinic is unique for its role in managing the province’s long-term care placement process and overseeing critical emergency response activities, including pandemic management and natural disaster evacuations.
