The Care Collective

The Care Collective was founded in Leslieville by three Primary Care Nurse Practitioners to expand healthcare access in East Toronto through a practice model centered on patient empowerment and professionalism. Their mission is to provide collaborative care where patients are educated and supported as active partners in managing their health. The team brings significant expertise, including individual providers with over 25 years of experience in specialized critical care and a decade in diabetes education. This nurse practitioner-led clinic is unique for its fee-for-service approach that prioritizes on-time appointments and cost-effective, high-quality primary care.
