Established in 2007 as part of an Ontario Ministry of Health initiative, the West Durham Family Health Team has over 15 years of experience providing collaborative, multidisciplinary care. Their mission is to enhance patient well-being through accessible, patient-centered services that emphasize providing the right care at the right time and place. The clinic is unique for its integration of state-of-the-art information technology to facilitate team communication and its active role in training the next generation of medical and healthcare professionals.
